WHAT IS UNDER EYE FILLER?

Under eye filler is a technique that uses hyaluronic acid based dermal fillers to fill the hollow space that forms beneath the eye with age. Filling in this hollow space results in less shadowing and brighter, more alert-appearing eyes. At SLK Clinic, we perform under eye correction frequently as a part of a greater anti-aging regimen.

Why Hyaluronic Acid?

Every tear trough filler we use is built on hyaluronic acid, a sugar molecule that occurs naturally in your skin and joints. HA binds up to 1,000 times its weight in water, so when injected, it holds moisture and adds volume where it’s placed.
HA fillers are safe because your body already recognizes the molecule. You’re not introducing a foreign protein or synthetic polymer. They’re also reversible: if you don’t like the result or experience a complication, an enzyme called hyaluronidase can dissolve the filler within minutes.
Over time, your body breaks down the HA gel through natural metabolic processes, which is why results last months rather than years. That gradual fade means you’re never left with a permanent change you didn’t want, and you can adjust the amount of filler at each appointment as your face changes with age.

Are you a good candidate for Under Eye Filler?

Signs that you might require tear trough/under eye filler include:

  • under eye hollowness
  • age 28 or older
  • dark circles beneath the eye
  • flattened facial shape below the eyes
  • volume loss in the cheek area
  • feels like just skin on bone below the eye without much padding

Occasionally filler may be used to disguise eye bags or protrusions of fat or skin beneath the eye. Botox also helps with certain types of under eye bags.

WILL TEAR TROUGH FILLER CORRECT CREPEY SKIN AROUND THE EYE?

Under eye filler may help improve the appearance of crepiness below the eye but other therapies like radiofrequency microneedling and Botox should be utilized in conjunction to achieve best results.

WHAT HAPPENS DURING TEAR TROUGH FILLER INJECTIONS?

First, the area under your eyes will be numbed with numbing cream or injectable lidocaine. Then the clinician will use a long, slender micro-sized straw to deposit the filler in the hollow space that needs filling. Occasionally needles are used in addition to cannulas.

The filler will be located beneath the skin and muscle. You will feel pressure and movement as the filler is placed. Occasionally you will feeling a “popping” sensation which is normal. The total injection time is about 5 minutes per eye. The numbing will wear off in about 1 hour.

Which Filler Brands Do We Use for Tear Troughs?

We choose from several hyaluronic acid filler lines. The two we reach for most often are Juvéderm Volbella and Restylane-L, though we also use products from the RHA Collection when patients need a filler that adapts to movement. Selection depends on your skin thickness and the degree of hollowing.

Juvéderm Volbella

Volbella has a smooth, cohesive gel that spreads evenly beneath thin under-eye skin. It’s less likely to create visible lumps and offers results that last up to 12 months. We often select Volbella when a patient has very delicate skin or when we’re layering filler for a natural finish.

Restylane-L

Restylane-L uses smaller hyaluronic acid particles, which makes it ideal for precise placement in the tear trough. It includes lidocaine, so injections feel more comfortable. Restylane-L integrates well with surrounding tissue and can last 12 to 18 months in the under-eye area.

Belotero Balance

Belotero Balance is engineered to blend into skin without leaving a noticeable edge, which is important in the thin under-eye zone. It’s a lighter filler that works well for patients with mild hollowing or when we’re correcting fine lines alongside volume loss.

RHA Collection

The RHA (Resilient Hyaluronic Acid) line is designed to move with your facial expressions. We sometimes use RHA 2 or RHA 3 in the tear trough when a patient has dynamic movement or when we want a filler that adapts rather than stays rigid. RHA products are newer and priced at a premium, but they offer flexibility and a natural look in motion.

IS THERE DOWNTIME AFTER UNDER EYE FILLER?

Occasionally there will be bruising so you should always base your social plans around the timing of your treatment just to be safe.  If you do get a bruise, it will take about 1-2 weeks for your body to metabolize the bruise. You can apply makeup to conceal any bruises starting 12 hours after your injection.

Usually there is no swelling at first, but after a few hours to one day you will feel a little bit puffy. The swelling is minor in most cases, but it is possible for swelling to persist for 1-2 weeks.

If you already suffer from swollen or puffy eyes that take half a day or a full day to resolve, you may not be a good candidate for under eye filler.

Yes, but only when dark circles are caused by shadowing from hollows. Filler fills the trough and lifts the skin, which reduces the shadow and brightens the under-eye area. If your dark circles are caused by pigmentation or visible veins rather than depth, filler won’t change the color. You’d need topical treatments or laser therapy for those concerns. We’ll assess your anatomy during the consult to see if filler will address your specific type of darkness.
