A few simple steps before your appointment reduce bruising and swelling. Stop taking blood thinners, aspirin, ibuprofen, and other NSAIDs (like Advil or Aleve) 48–72 hours before your visit, if your doctor says it’s safe to do so. These medications thin your blood and make bruising more likely. Skip alcohol for 24 hours beforehand for the same reason. If you take a prescription blood thinner, check with the prescribing doctor before pausing it. Arnica supplements or topical arnica gel can help minimize bruising; start them a day or two early if you like. On the day of treatment, arrive with clean skin. Your injector will numb the area with a topical cream before placing the filler, so discomfort is minimal.

How Hyaluronic Acid Fillers Work

Hyaluronic acid is a sugar molecule your body produces naturally in skin and connective tissue. When we place a hyaluronic acid gel into the lips, it attracts and binds water molecules (up to a thousand times its weight), which is how it creates volume. The gel integrates with your lip tissue over the first few days, and the swelling you see immediately after treatment is partly the filler itself and partly the water it pulls in. That’s why some fillers, like Juvederm Ultra, swell more than others: they’re more hydrophilic. Over the following months, your body’s enzymes slowly break down the hyaluronic acid chains, and the volume fades. The filler doesn’t migrate elsewhere; it metabolizes into water and carbon dioxide and leaves your system. Because the molecule is identical to what your body already makes, allergic reactions are rare. If you don’t like the result, an injector can dissolve hyaluronic acid filler with an enzyme called hyaluronidase, usually within a day or two.

Why Lips Thin With Age

Lip volume peaks in your twenties and starts to decline in your thirties. The main reason is collagen loss: your body makes less collagen each year, and the collagen fibers already in your lips break down faster than they’re replaced. Connective tissue loosens, so the lip border becomes less defined and the vermilion (the pink part of your lip) loses its crisp edge. The fat pads inside your lips shrink, too, and the skin itself gets thinner and less elastic. Bone resorption in the upper jaw can make the distance between your nose and upper lip longer, which flattens the lip further. Sun exposure accelerates all of these changes, especially the vertical lines that form above the upper lip. Smoking does the same. Hormonal shifts during menopause speed up collagen loss, so many patients notice a sharper drop in lip fullness in their late forties and fifties. Filler replaces some of that lost volume and re-defines the border, but it doesn’t stop the underlying aging process.

Lower Lip Focus for a Subtle Pout

If you want a bit more fullness without looking obviously treated, ask your injector about a lower lip focus. This technique places most or all of the filler in the lower lip, leaving the upper lip untouched or adding just a small amount for balance. The result is a soft, understated pout. The lower lip looks naturally plump, and the upper lip keeps its original shape. It works especially well if your lower lip is already thinner than your upper lip, or if you’re concerned that adding volume to the upper lip will look overdone. We usually use a softer filler like Volbella for this approach, because the goal is a natural feel and minimal projection. The technique also reduces swelling, since you’re treating a smaller area. It’s a good first step if you’re new to filler and want to see how you like the change before committing to a fuller treatment.

I have thin lips, is there a specific strategy?

Yes. Thin lips benefit from a layered approach that builds volume gradually rather than all at once. We often start with Volbella, which is the softest filler and blends into thin tissue without looking overfilled. It hydrates the lip and makes the border a bit more defined, but the result stays natural. If you want more structure or a crisper Cupid’s bow, Vollure is a better choice. It adds shape and holds a firm edge, even in tissue that doesn’t have much volume to start with. Either way, we usually place a conservative amount in the first session and let you see the settled result over two weeks. If you want more, we add another layer in a follow-up visit. Thin lips can’t absorb as much filler in one sitting as naturally full lips, so rushing the process can lead to a tight, unnatural look or filler migration outside the lip border. Building slowly gives you control over the final shape and keeps the result proportional to your face.

Correcting Lip Asymmetry

Most people have some degree of lip asymmetry: one side slightly fuller than the other, or a Cupid’s bow that sits higher on one side. Filler can balance that. Genetic asymmetry is the most common case we see. Your lips simply grew that way, and no injury or other cause explains it. Injury-related asymmetry shows up after trauma, Bell’s palsy, or surgical scarring that pulled one side out of position. Either way, the correction strategy is the same. Your injector maps both sides of your mouth and identifies where volume is missing or where the border has lost definition. We place small amounts of filler in the thinner or shorter side to match the fuller side, rather than adding volume everywhere. The goal is balance, not maximum size. Sometimes we’ll add a tiny bit to both sides to keep the result symmetrical, but the bulk of the filler goes where it’s needed. Asymmetry correction often takes two sessions, because it’s hard to judge perfect balance while the lips are still swollen. We get you close in the first visit, then fine-tune after everything settles.
